COORDINARE Annual Review 2021/22

It gives us great pleasure to share our annual review which details our key achievements for 2021/22.
Expansion of our innovative social prescribing service across the region, and codesign of a care coordination service with local stakeholders were two exciting new initiatives.
Only by working together with a clear and shared vision, strong local partnerships, and a team committed to practical collaborative actions are we able to create change for local people and communities.
This year we strengthened our commitment to reconciliation with the release of our Innovate Reconciliation Action Plan.
Through a second year of the pandemic, our team has worked proactively with our partners - establishing new mental health services and ensuring vaccinations and care for the most vulnerable in the community.
As this is the last annual review under the leadership of our current CEO, Dianne Kitcher, we express our gratitude for her invaluable leadership, and unwavering dedication to support of the primary health model and the broader health system throughout her seven-year tenure. Ms Kitcher led the start-up of COORDINARE and her commitment to our vision and purpose leaves a benefitting legacy to our organisation, the PHN network more broadly and ultimately our communities.
