COORDINARE Annual Review 2022/23

We are proud to share our annual review which highlights progress against our 2021-2023 Strategic Plan - ‘Transforming health together’.
Over the past year our team has worked proactively with our partners, ensuring access to health care for the most vulnerable in the community. Expansion of our innovative social prescribing service into the southern region, and co-design of a care coordination service with local stakeholders were two exciting new initiatives.
We continued to support general practice to embed quality improvement processes and to respond to the COVID-19 pandemic and care for the most vulnerable in the community. We are in the process of establishing a number of Medicare Urgent Care Centres (or UCCs) across the region to care for urgent but non-emergency health needs, like fevers, broken bones and mild respiratory illnesses.
To improve the reach and impact of mental health services over the last year we’ve commissioned eight new mental health services for adults, in addition we’ve commissioned two services for youth being headspace in Kiama and Shellharbour all of which will be fully operational in the upcoming months.
The launch of the Bega Valley Eurobodalla Suicide Prevention Collaborative in May was a significant milestone for our region. We proudly fund the backbone team of this new alliance as well as the Illawarra Shoalhaven Suicide Prevention Collaborative, who are working together to lower suicide rates within the region.
We worked closely with local communities and key stakeholders to understand local needs for older people and are well prepared for the increased role that PHNs will have in implementing the government’s response to the recommendations of the Aged Care Royal Commission.
We are pleased to have our Aboriginal Health Operational Advisory Group set up and offering us their advice and wisdom and we have continued to implement our Innovate Reconciliation Action Plan.
None of these achievements could happen without the support of our Board, staff, health care providers and stakeholders, friends and community representatives, who have continued to contribute to our work to improve the health of our community over the past year.
